We know you have many choices when choosing a Dentist in Forest Hills, NY so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Insurance and Billing
We accept most traditional insurance plans, contact our office to verify acceptance of your plan. Gregory Mark, D.D.S. does not participate in Health Management Organizations; however, we will be happy to file your insurance claims for you. We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.

Financing Options

Our office does not participate in Health Management Organizations; however, we will be happy to file your insurance claims for you. As your treatment at our office progresses, you may encounter the necessity of the certain services (such as annual deductible, co-payments due to your dental policy, differences in the fees, use of better quality materials, etc.) that will not be covered by you insurance carrier. We would like to ask you to choose the best method of payment for such cases from the further shown options. Your choice allows us to create a payment system, which will fit your needs as closely as possible.

Please, let the us know which of those options is more suitable to you. As you are examined by the one of our doctors, you will be offered to see our office manager who will coordinate with you your future treatment if such is needed. She will help you to maximize what your dental plan offers as well as will she answer any financial question that may arise as your treatment plan is created.
